The AfterCare Program Case Manager is responsible for the management of a caseload of families living in the scattered-site AfterCare Housing Program. The AfterCare Program Case Manager provides range of supportive services that assist families in maintaining permanent housing. This includes budgeting, renter’s rights and responsibilities, rental listings and applications. The AfterCare Program Case Manager monitors family stability and goal attainment; refers family to community resources; serves as a liaison between property managers/ landlord, government agencies and the families as needed. The AfterCare Program Case Manager reports directly to the AfterCare Program Manager.
