The Health Trust is looking for a Case Manager for the HIV/AIDS Services Department. The Case Manager assesses the needs of individuals with HIV/AIDS and connects them with internal services and/or community resources; assists clients with accessing and/or troubleshooting public/private benefits and entitlement programs; coordinates with staff and other providers (HIV and non-HIV related) to attempt to get client needs met and to avoid duplication of services; provides outreach to clients who are out of care, and outreach to the community.
EXPERIENCE AND TRAINING
Graduation from an accredited college or university with a Bachelor’s degree or Masters Degree in Health, Human or Education Services and one year of case management experience with HIV positive persons, and/or persons with a history of mental illness, homelessness, chemical dependence and experience with families preferred.
Bilingual Required; English and Spanish or English and Vietnamese preferred.
KNOWLEDGE, SKILLS, & ABILITIES
Knowledge and experience with public/private benefits and entitlement programs.
Knowledge of HIV/AIDS. Adept at establishing rapport with clients, families, health care professionals, and community service organizations.
Ability to provide culturally competent and linguistically appropriate services.
Ability to establish and maintain appropriate professional boundaries, and to maintain a high standard of professional and ethical conduct.
Strong oral and written communication skills to perform essential duties above and to communicate effectively with diverse populations.
