The goal of the City of San Jose Rapid Rehousing Program is to provide housing stability to homeless individuals, and households in Santa Clara County, CA. We achieve this goal by providing participants with comprehensive supportive services to help them attain permanent housing including outreach and engagement, intensive, individualized case management services, housing search, and temporary financial assistance.
DUTIES/RESPONSIBILITIES: Direct Services (70%)
Conduct Outreach to locate referred individuals, determine eligibility, and enroll as appropriate
Maintain a caseload providing intensive case management and other direct services to program participants.
Provide community-based case management services to program participants
Refers participants to eviction defense legal services as appropriate
DUTIES/RESPONSIBILITIES: Documentation (20%)
Documents, via progress notes in HMIS, all case management activity within 3 business days and maintains all other case file paperwork and documentation
Participates in ongoing analysis of and program alignment with industry best practices
Proactively and consistently improves the quality and effectiveness of program services
Conduct and document ongoing re-assessments of the participant's needs and status
