The goal of HomeFirst’s case management services at Sunnyvale shelter is to provide access to resources and housing for chronically homeless individuals and families in the Northern part of Santa Clara County, California. The case management services will achieve this goal by providing participants with comprehensive supportive services to help link them to a network of community resources- psychiatric, medical, employment, housing, income, and other services.
DUTIES/RESPONSIBILITIES:
Maintain a caseload as a case manager of persistently homeless individuals in North Santa Clara County by providing direct case management to clients
Provide community-based case management services to program participants
Work with other team members as applicable to communicate interventions with the client and to link them to additional services
Collaborate with community partners regarding support services for clients

QUALIFICATIONS:
Bachelor’s degree + one-year minimum human service experience, or minimum of three years human services experience
Bilingual Spanish speaking, required
Strong written and oral communication skills
Experience in developing relationships with community partners and outside agencies
