The part time Client Support Specialist for the LGBTQ Transitional Living program must have lived experience and be passionate about helping LGBTQ clients connect with appropriate services and role modeling appropriate behavior.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Engage youth in learning about forming supportive peer communities by facilitating/co-facilitating workshops, community-building groups, and other special events.
Assist CM with program orientations, intakes, and assessments utilizing AWARDS Electronic Health Record system
Enter client information and updates in the AWARDS electronic heath record within required timelines
Engage with all youth in program using motivational interviewing skills and trauma informed care and support youth in evaluation/quality improvement activities in achievement of program outcomes and contract requirements
Assist in running focus groups with youth
Foster regular communication with Case Manager and Program Manager to ensure up-to-date information sharing regarding clients and issues affecting the LGBTQ community.
Collaborate with program staff to support meaningful engagement of youth participants in program activities
Provide crisis intervention and conflict de-escalation as needed.
