The Clinician II provides youth and families with behavioral health services utilizing the Transformation Care Planning (TCP) model in collaboration with client’ school placement to improve the client’s emotional wellbeing.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide mental health intake & assessment, treatment planning, therapy and case management to youth and families in-home & on-site
Maintain SCC and Medi-Cal standards and deadlines
Must be able to efficiently travel to client schools/ homes for appointments (providing modified services at this time to honor Shelter in Place order)
Assist youth and families in skills-building to improve life domain functioning and developing appropriate parenting skills as well as educational and vocational plans
Master’s Degree in Counseling or other Behavioral Science and current BBS registration required.
Medi-Cal Mental Health program preferred but not required in Santa Clara County.
Ability to balance excellent client care with meeting deadlines and ensuring positive program goals and outcomes. Experience providing services to clients of various racial, ethnic, socioeconomic backgrounds or sexual identity who are overrepresented in homeless populations as well as working in different cultural environments. Bilingual (Eng/Spanish) communication skills required.
