The goal of the program is to connect participants with services that will result in permanent housing. The Citywide Outreach and SOAR team includes Outreach Specialists, Case Managers, Drug and Alcohol Counselors, Clinicians and Clinical Supervision, Program Manager and Director.
Provide drug and alcohol counseling to unhoused individuals at targeted encampments throughout the city of San Jose Conduct comprehensive assessment of participants use history and present needs.
Work with program participants to develop and implement an individual treatment plan, and provide support to help them achieve their goals.
Meet regularly with participants at a frequency determined by their needs and contract requirements.
Conduct ongoing re-assessments of the participant’s needs and shift services appropriately.
Provide referrals to internal Case Manager for additional community services requested.

Education and Experience Required Drug and Alcohol Counselor Certification BASW preferred
Skills, Abilities, and Knowledge Strong verbal and written communication skills
Proficient with Microsoft Office and general computer skills
Understanding of and sensitivity to the needs of the unhoused and those with criminal backgrounds
Understanding of Housing First and Harm Reduction principals Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks
