Job Overview: Asian American Recovery Services/HealthRIGHT360 (AARS / HR360) is looking for a committed Case Manager that will service clients in our Youth Mental Health Programs, Project H.E.R.E. (Healthy, Empowered, Resilient, Evolving) and Project F.O.R.W.A.R.D. (Finding Our Resilience Within And Reaching Dreams) who is passionate about working with clients with mental health issues (as well as co-occurring disorders) by providing targeted case management services. This position works with key stakeholders (e.g., Clinician, Clinical Program Manager, Clinical Supervisors and Psychiatrist) in coordinating quality care for clients. Office hours of operation are Mondays and Fridays from 9:00AM to 6:00pm; Tuesdays, Wednesdays and Thursdays from 9:00AM to 8:00PM. Reliable transportation required.
Key Client Care Responsibilities: Provides targeted case management services to both English and Spanish speaking clients. Facilitate and support clients’ movement throughout the program with particular attention to need of ancillary resources and progress toward meeting goals outlined in treatment plans. Facilitates orientations, coordinates care with client and key stakeholders, may assist providing client education. Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status. May support client at external appointments. Participate in development of discharge plans and levels of care including extension requests based on need. Treatment Setting Responsibilities: Facilitates targeted case management as appropriate in the community and at agency site as assigned. Conducts community outreach and education workshops. Attends required trainings and meetings. Plans and facilitates client celebrations and special events as assigned. May be asked to work on a weekend or during after-hours for crisis stabilization, home visits, community education, resource fairs, and special events.
Documentation Responsibilities: Collaborates with each caseload client and other available internal and external resources in support of the health and recovery needs of the client. Properly documents all case management sessions and attempts to contact in a timely manner. Also, maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements. Will participate in Peer Chart/ Utilization Reviews for preparation of program audits. Will adhere to documentation policies and procedures set forth by Compliance Department. Be responsive and pro-active in all correspondences with referring parties, judges, probation, social workers, and the Quality Improvement Dept. via treatment status reports, phone calls, faxes, etc. Will provide informed, written status reports to the above entities as appropriate.
