The Health Trust has an amazing opportunity for a Lead Intensive Case Manager to join the Housing Services Department.
The Lead Intensive Case Manager (LICM) holds three primary roles:
Providing clinical support to the staff on the intensive case management (ICM) team
Carrying out quality improvement (QI) processes and assurance (QA) protocols for the ICM team
Providing direct services to assigned clients
For the clinical support role, the LICM will support the staff on the ICM team on client engagement, assessment, rehabilitation services, treatment plan development, and documentation meeting medical necessity for MediCal reimbursement. For the QA role, the LICM will work with CCP-ICM Program Manager (PM) to develop and carry out QA protocol. For the direct services role, the LICM will provide direct services to an assigned chronically homeless client caseload in various settings to stabilize health and well-being, promote permanent and stable housing, and improve quality of life.
EXPERIENCE AND TRAINING
Licensed clinician in good standing with the CA Board of Psychology and/or Board of Behavioral Sciences, 2 years post licensed preferred.
Licensed clinician <2yrs post-license and/or license completion w/in 1 yr will be considered.
Graduate education completion from a regionally accredited program.
