Job Description
The Homeless Prevention Services Case Manager provides direct support to low-income individuals and families to help them maintain stable housing and avoid homelessness. This includes comprehensive case management, resource navigation, safety-net services, and community outreach. The role requires strong coordination with internal programs and external partners to ensure clients receive timely and effective assistance.
