Case Manager for Self-Determination Services
Job Description
Job Summary: While maintaining good public relations and implementing the goals and vision of the regional center, this position is responsible for providing and coordinating services to consumers in accordance with the provisions of the Lanterman Developmental Disabilities Services Act, and policies established by the San Andreas Regional Center Board of Directors. Approximate caseload of 40 -50 with, a special emphasis on working with individuals in the Self-determination program.
Helps individuals and families begin and/or continue with the Self Determination program including: Creating the Certified Budget each year Creating the Spending Plan Developing IPP objectives related to Self Determination
Requirements:
- BA/BS in Social Work
- Minimum of two years of experience
