Job Description

Function: Assesses clients’ needs and facilitates their usage of appropriate Health and Social Services. Provides case management and general counseling Services to clients and their families. This position reports to the Social Services Administrator.

Hours: 32 hours per week

Rate of Pay: $27/hour - $31/hour depending on experience

Duties & Responsibilities: • Assess needs and evaluate problems of clients and their families; help navigate appropriate resources. • Provides case management services including home and hospital visits. • Develops care plan & goals, places wellness calls and monitors clients’ progress. • Plans and facilitates support groups for clients, and workshops and discussions as needed. • Assists clients in the utilization of existing community resources. • Refers cases to and consults with other agencies and community organizations. • Plans and facilitates workshops and discussion/support groups for clients. • Works closely with Senior Day Services program including identifying potential clients, making referrals, and making assessments. • Maintains case records, completes case reports and handles correspondence as required. Responsible for confidentiality of client records. • Performs any other duties deemed necessary for the smooth operation of the agency.

Education and Experience: • Bachelor’s Degree or higher-level education preferred.

OR

• Combination of education and experience in lieu of degree.

Qualifications: • Demonstrated experience in planning and delivering group workshops and support sessions to meet program objectives. • Familiarity with available community resources. • Must be able to read, write, and speak English; Bilingual in Japanese, Vietnamese, Spanish, or Chinese is a plus. • Effective written and oral communication skills. • DOJ (Dept. of Justice) and FBI Fingerprint clearance.
• Must have a health screening and TB test that is not more than a year old from date of hire and no later than seven days after employment. • Travel Required – must have Valid California Driver’s license & proof of insurance or willingness to Rideshare. Mileage and travel expenses will be reimbursed. • Current First Aid and CPR certification. • Knowledge of or interest in the API/Japanese/JA community and culture, and its elders. • Minimum Age Requirement: 18 years of age.

Located in San Jose’s historic Japantown, Yu-Ai Kai is a nonprofit senior center with a variety of services for seniors (50+) and their families. Our primary service areas include: Social Services, Senior Day Care (socialization), Nutrition and Activities. Programming is designed around our Healthy Aging Initiative.
